    These terms explain how you may use the FORM 300 LOGBOOK recordkeeping workspace, public resources, subscriptions, records, downloads, and service limits.

    1. Acceptance and eligibility

    By visiting, creating an account for, or using FORM 300 LOGBOOK, you agree to these Terms of Service. If you use the service for a company or other organization, you confirm that you are authorized to accept these terms for that organization.

    You may use the service only if you can legally enter a binding agreement and your use complies with applicable law. If you do not agree with these terms, do not use the workspace.

    2. Account, sign-in, and security

    Keep the account and contact information you provide accurate and current. Protect your password, sign-in information, devices, and recovery methods connected to your account. Use the available password recovery process when you need to regain access. Do not share access in a way that bypasses account controls or permits unauthorized use.

    The workspace is private to the account that is authorized to use it. You are responsible for activity under your account and for deciding which people in your organization may access information through your account. Contact your account provider and FORM 300 LOGBOOK promptly if you believe someone accessed your account without permission. We may ask for information needed to confirm account ownership before acting on a request.

    3. What FORM 300 LOGBOOK provides

    FORM 300 LOGBOOK provides a public website and a private recordkeeping workspace. The public site includes marketing pages, pricing and checkout information, account and password pages, payment status pages, educational OSHA recordkeeping guidance, frequently asked questions, and public blog articles. Those materials are informational and do not create legal or advisory services, OSHA filing services, or a guarantee that a workplace is compliant.

    Free resources and sample files

    The site may offer a free OSHA recordkeeping review checklist and a fictional sample export pack. A checklist request does not require a paid workspace subscription. The fictional sample pack is available after its request flow without an account or payment. The requested files are revealed or downloaded in the browser. The sample pack uses fictional information, is for review and learning, and is not a complete private workspace, legal advice, an OSHA submission, an ITA upload file, or a compliance determination.

    Public guidance, the checklist, the sample pack, the blog, and other free resources are provided for general information. Review current official OSHA materials and obtain qualified advice for decisions about your workplace. Resource availability and content may change.

    FORM 300 LOGBOOK is independent software. It is not OSHA, is not affiliated with OSHA, and is not sponsored, endorsed, approved, or certified by OSHA. It does not represent OSHA, is not an OSHA filing service, and does not submit, file, or transmit records directly to OSHA. It is not operated by or a representative of OSHA or the U.S. Department of Labor. The service is not legal advice, current official OSHA guidance, or a guarantee of compliance.

    Forms 300, 300A, and 301 have different purposes. Form 300 is a running case log, Form 301 captures incident detail, and Form 300A summarizes a calendar year's recordable cases. They are not all required for every incident or every workplace. You must determine which requirements apply to your organization using current official guidance or qualified advisors.

    4. Your records and responsibilities

    You keep your ownership of the information, text, and other material you enter or provide. If you provide information about employees, customers, or another person, you must have the authority and permissions needed to use that information for your organization's recordkeeping work.

    The private workspace can let you switch between workplaces and filing years, save drafts, and connect related Form 300 cases with Form 301 incident details. It can also prepare Form 300A annual summaries, retain certification and review details, and keep quarterly review checkpoints and history associated with the selected workplace and filing year. Review history can include a review date, next planning date, cadence, reviewer name or title, notes, and checklist confirmations. These entries are planning and recordkeeping aids. They do not certify a record, send an automatic reminder, or file anything with OSHA.

    The workspace can create populated PDF copies of Forms 300, 300A, and 301 for review or archiving, readable CSV backup or archive files, and separate OSHA ITA-format CSV preparation files when available. These outputs are preparation materials, not official OSHA forms and not direct filings. You must review every field, calculation, certification, and downloaded file against current requirements. If an ITA-format file is appropriate, you are responsible for removing anything that should not be submitted and manually uploading it through OSHA's current ITA system. FORM 300 LOGBOOK does not log in, transmit, or file records with OSHA.

    You are responsible for:

    • providing accurate, complete, and current workplace, employee, incident, annual summary, certification, and review information;
    • deciding whether your workplace or an incident is covered and which forms, fields, classifications, and deadlines apply;
    • reviewing every entry, connection between forms, calculation, certification, review checkpoint, and downloaded copy before relying on it;
    • keeping backups of records and exports your organization needs, including copies you download from the browser;
    • meeting any requirement to retain, post, report, disclose, certify, or submit information; and
    • completing any required manual upload or other government action outside the service.

    You give FORM 300 LOGBOOK a limited permission to host, store, transmit, reproduce, and process your records as needed to provide the workspace, save and connect entries, create requested downloads, protect accounts, support the service, and comply with law.

    5. Subscription pricing, recurring billing, and cancellation

    The current displayed introductory subscription prices are $79 per month or $708 per year. Annual billing is equivalent to $59 per month and reflects $240 in annual savings compared with twelve monthly payments at $79. The amount and billing interval you select are shown during checkout and control your subscription.

    Subscriptions renew automatically at the selected interval until cancelled. Use the billing portal available through the workspace to cancel or manage billing. Cancellation stops future renewals. The current paid period remains available through its paid period, subject to these terms and service availability, and cancellation does not reverse a completed billing period.

    No refunds after billing.

    There are no refunds after billing and no credit for unused time. Completed charges are not refunded or credited after billing, except where applicable law requires otherwise.

    We may change pricing at any time. A pricing change applies to future billing periods, not a period that has already been paid, and we will provide notice where required. Stripe may display and process taxes or other charges that it applies at checkout.

    6. Acceptable use

    Use the service for lawful recordkeeping work that you are authorized to perform. Do not interfere with the service, probe or bypass access controls, copy it for resale, reverse engineer it except where applicable law allows, impersonate another person, upload malicious material, or use another account without permission.

    Do not treat a workspace entry, review checkpoint, resource, or download as a substitute for your own review. You decide what information belongs in a record, how it should be classified, and what your organization must retain, post, submit, or disclose.

    7. Intellectual property

    FORM 300 LOGBOOK, its software, design, text, branding, public resources, and other service content belong to the service owner or its licensors. These terms give you a limited, non-exclusive permission to use the service and its resources for your organization's recordkeeping work while you are allowed to access them.

    That permission does not transfer ownership and does not allow you to copy, modify, distribute, sell, lease, sublicense, or create a competing service from the service or its content. Your records remain yours, subject to the limited permission in these terms.

    8. Availability, changes, suspension, and limits

    The service, public resources, downloads, and workspace features may be updated, changed, paused, or discontinued. Maintenance, provider issues, security events, legal requirements, or circumstances outside our control may affect availability. We do not promise uninterrupted or error-free operation, permanent storage, or that every output will reflect every current rule.

    We may suspend or end access for nonpayment, misuse, a security or legal risk, or if the service is discontinued. When practicable, we may provide notice, but notice is not always possible. Keep independent copies of records and exports that your organization needs.

    9. Disclaimers, liability, and governing law

    To the extent permitted by law, the service, public information, resources, and downloads are provided as available, without promises that they will meet every particular business or regulatory need. We do not promise that information entered by a user is complete or that an output is current, accurate for every situation, or sufficient for compliance. Nothing here removes a right that applicable law does not allow us to remove.

    To the fullest extent permitted by applicable law, FORM 300 LOGBOOK will not be responsible for indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, business, records, or data, arising from or related to the service. Where a limit is allowed, our total liability for a claim related to the service will not exceed the amount you paid for the service during the 12 months before the event giving rise to the claim.

    These terms are governed by the laws of the State of New Jersey and applicable United States federal law, without regard to conflict-of-law rules. Subject to applicable law, disputes related to these terms or the service will be handled in the state or federal courts located in New Jersey. If a court finds one provision unenforceable, the remaining provisions will continue to the extent allowed by law.

    10. Changes to these terms and questions

    We may update these terms as the service, pricing, public resources, or legal requirements change. The updated version will be posted on this page with a new effective date. Your continued use after an updated version becomes effective means you accept the revised terms.
